Correction of the Inverted Nipple with Multiple Z-plasties

Abstract
The inverted nipples have been treated by lots of different methods by many authors, but there were no golden standard rules. The purpose of this study is to introduce a simple, reliable method for correction of inverted nipple and to present our clinical experience. We performed 34 corrections of the inverted nipple using multiple Z-plasties. The mean follow up period was 10 months, and the mean patient¡¯s age was 33 years old. Trans-nipple incision was made from 3 to 9 o¡¯clock on the nipple with traction sutures. This incision was carried deep into the breast tissues. Fibrous band was cut and released by Z-plasties. Then interposition of the surrounding tissues with Z-plasty flaps was made to fill the dead space and maintain the height of nipple. The traction sutures were maintained at the base of nipple for 1 week after surgery to avoid trauma and pressure on the nipple. These suspension sutures were anchored to the top of the customized syringe. The mean projection of nipple was 5.2 mm in 34 cases. Our new technique leaves a minimal scar and sensory change after the surgery. In addition this technique is very simple and takes short operating time with reliable reproducibility.
